Supervisor:  Beth Hess, LCSW-C  Holocaust Grant Administrator: Ruth Kershner

A social service and nursing staff that have the experience and skill to provide the best care possible to help elders live safely, with dignity,and promoting independence.
Case management, counseling, home health care including personal care assistance and light housekeeping (through Premier Homecare), in-home Transitions and Hospice care, escorted transportation to medical appointments, kosher meals on wheels, purchase of medicines or assistive devices and medical alert system,  volunteers for shopping, friendly visits and other assistance. 
Care management includes comprehensive evaluation of needs, consultation with family, ongoing planning and management, individual psychotherapy and family counseling including support groups, assistance in choosing living facilities and nursing homes, advocacy and liaison assistance, and resource identification and referrals.
Grant supplies needed services for Holocaust survivors.
Premier Homecare provides home care aides, in home mental health services and companions for as little or as much time as you need - daily, weekly or around the clock with a minimum of two hours. 301-984-1742.

Fees: As applicable. Can be adjusted according to income. Psychotherapy may be covered by Medicare, Medicaid or insurance.
